Here's what's in the doctor's note look like

In the medical world, doctors have important administrative duties, among others, to issue a certificate and are part of medical practice, so its existence is absolutely the authority and responsibility of doctors, just like a doctor's responsibility in establishing a diagnosis.

This note is used by the community for certain purposes according to their needs, Doctoral notes have different forms and variations, the following are things that must be in a doctoral note:

• Doctor/Medical Facility Information: Include the name, address, and contact information of the doctor and/or facility.
• Date: Fill in the date and time (if known) of the doctor's appointment.
• Patient Information: At a minimum, include the patient's full name.  You may also want to add your age, gender, date of birth, and address.
• Purpose : Include the reason for medical visits for example, cold, flu, accident, injury, etc.
• Requirement Certification: This is a medical professional statement certifying a patient's illness, injury or other specific health condition.
• Who Authorized: State how many days the patient must be absent, the type of work or play they are physically able or unable to do, or any other activity they are permitted to do.
• Signature: The doctor's signature will appear at the bottom of the page above his full name and title.  Some recipients may also require the signature to be notarized.
